According to CHP on October 21, 2016 at approximately 4:25 PM Trista R. Hoffman (16) of La Grange was driving a 2002 Lexus southbound on La Grange Road, north of Bonds Flat Road.
Dorothy Eisemann (66) of Copperopolis was
driving a 2009 Toyota Camry northbound on La Grange road north of Bonds flat Road.
An unknown vehicle was traveling northbound on LaGrange road overtaking Eisemann in the Toyota, passing over solid double yellow lines.
Hoffman swerved to the right to avoid a head-on collision with the unknown vehicle., traveling partially onto the west dirt shoulder of LaGrange Road.
As Hoffman turned her Lexus back onto the roadway, she lost control of the Lexus as it traveled sideways in a southeasterly direction.
The Lexus overturned onto its right side and the roof of the Lexus collided with the front end of the Toyota.
The right front passenger in the Lexus was ejected from the vehicle and sustained fatal injuries. The name is being withheld pending notification .
The right front passenger of the Toyota, Rheinholt Eiseman (72) of Copperopolis was flown to Sonora Regional Medical center where he succumbed to his injuries.
Trista Hoffman, the driver of the Lexus, sustained fatal injuries.
The right rear passenger of the Lexus, Dillon Hoffman (17) of La Grange, sustained major injuries and was flown to Modesto Memorial.
Dorothy Eisemann, driver of the Toyota was transported to Modesto Memorial via ground ambulance with minor injuries.
Left rear passenger of the Lexus, Annie Johnson, sustained major injuries and was flown to UC Davis Medical Center.
The driver of the unknown vehicle fled the scene northbound from the collision.
Alcohol and drugs are not suspected and the collision is still under investigation. CHP strongly urges anyone that witnessed this collision to please call Officer Pulido at 916-634-1502 or 209-984-3944.
