Nominations for Senior of the Year

   The public is now invited to make nominations for 2013 Senior of the Year.

    Recognizing that senior volunteers are a vital lifeline for most organizations, the Calaveras County Commission on Aging is now accepting applications to honor those that provide these valuable services in Calaveras County.

     To nominate a candidate, complete the information below using your organization's letterhead.

   As only information submitted will be used to select the winners, the Nominators are encouraged to work with those they nominate to ensure all volunteer work is captured for the Nominee.

   The more documentation, the better, as this will give the Commissioners a better chance to get to know the Nominees.

   To be eligible, nominee must:
  
    * live in Calaveras County
   
     * be 60 years of age or older

     * not have been a previous winner

   All entries must be typewritten and include:

    * your name, address, phone number and email address

    * nominee's name, address, phone number and email address

    * include # of years volunteering with what orgainization, detailed contributions

   *  List the reasons for nominating this person for Senior of the Year. Please be specific and list all the volunteer contributions this person has made in Calaveras County and not just those in your organization.

    * Do not use abbreviations or acronyms.

    *  All nominees will be honored with a Certification of Appreciation and acknowledged by their District Supervisor at the luncheon to be held on May 14, 2013 at St. Andrews Church in San Andreas.

    Please return information to :  Calaveras County Commission on Aging  891 Mountain Ranch Road # 20 San Andreas, CA 95249 by the deadline on Monday April 1, 2013.

   Commission on Aging members are not eligible to be nominated nor may they nominate a senior.
